Arizona's Firewise Recognized Communities   Map    Table

These two forms are used to submit application for recognition as a Firewise Community.
They are available as MS Word format for download and completion off-line.
1. Initial application form. This form is filed upon a communities initial application for recognition.
2. Renewal form.This form is required annually to maintain Firewise Communities/USA recognition.

    Assessment and Planning Process:

    The Arizona State Forestry Division has implemented a policy  outlining qualifications certification as an Advisor or Assessor.

    Forestry Division staff  can assist you in achieving the required fire observation experience and qualifications.

    This program is limited to persons from cooperating agencies, fire departments, consulting foresters, and community Firewise boards.

    There are two AZ Firewise trailers available to fire management agencies, fire departments and Firewise groups in AZ to aid in the presentation of FIREWISE education programs.
