Emergency Equipment Rental Agreements for vendors and contractors:
Below for your information and completion are links to the Arizona State Forester’s
Emergency Equipment Rental Agreement Package, including the following documents:
Important changes in this package:
Insurance: Please read Appendix A carefully, some insurance limits and language
have changed. Emergency Rental Agreements will not be considered valid unless
a current certificate of insurance meeting these standards is on file with the
State Forester.
Training: The State Forester will require a copy of Training Verification
(State Form 123) for all contractor NWCG Engine Bosses (ENGB.) responding
under this agreement. Emergency Rental Agreements will not be considered valid
until valid State Form 123 or equivalent for the ENGB(s) is on file with the State
Forester. In addition the contractor will need to maintain training files with Form 123
or equivalent on all other employee’s needing NWCG certification.
MAILING: Please send an original completed Form 100 and required insurance
and training documentation to your District Forester. An electronic version (e-mail)
would also be appreciated.
